Bill the moment work changes owner and keeps its context.

Midfleet should not charge for failed progress. An accepted handoff means the next agent or operator has taken ownership with the necessary state, scope, blockers, approvals, and evidence attached. Rejected or blocked handoffs remain part of the runtime record, but they do not consume paid usage.
